My Practice Profile allows you to submit updates to your demographic data and complete the quarterly attestation, confirming the accuracy of the information. We accept retroactive referrals within 5 calendar days of the first specialist visit. You can use the Referrals tool in the UnitedHealthcare Provider Portal to submit referrals. To access Referrals, visit UHCprovider.com and Sign In at the top-right corner. Then, select Referrals from the top bar.
