Join our network - Hospitals and facilities

Join the UnitedHealthcare network of health care facilities and hospitals. This page will walk you through the process of credentialing and provide resources.

Thank you for your interest in becoming part of a network of health care professionals and facilities committed to helping people live healthier lives and making the health care system better for everyone. 

 

How to request participation 

Submit your request to join our network through the UnitedHealthcare RFP portal.

Please note: Health care professionals employed by Federally Qualified Health Centers (FQHCs) and Rural Health Clinics (RHCs) should use the practitioner enrollment form to submit a request for each practitioner, not the Facility RFP portal; the Facility RFP portal relates to facilities only.

A complete request to join our network includes:
  • Proof of malpractice and general liability insurance
  • W-9
  • Phone and fax number
  • Email address
  • Physical facility address, including suite number, if applicable
  • ZIP code + 4
What happens after submission:
  • A representative will start the process with you
  • You’ll be registered in our system (if you’re not already)
  • You’ll receive a confirmation of your participation request – this usually occurs within 5 business days
  • We’ll quickly review your application; if we’re not accepting new applications for your area or specialty, we’ll let you know

To check the status of your credentialing application, Sign In with your One Healthcare ID. This allows you to access self-service tools or connect to a chat advocate for support. Please have the care provider’s full name, Tax ID number (TIN) and National Provider Identifier (NPI) number available.
